M4V is Apple’s slight variation on MP4. Technically the file is an MPEG-4 Part 14 container, just like any .mp4 — but Apple used the .m4v extension in the iTunes Store to mark purchased and rented video. DRM-protected movies and TV shows shipped as .m4v with Apple’s FairPlay encryption; unencrypted .m4v files also exist and are indistinguishable from .mp4 apart from the extension.
Most modern tools treat .m4v as a regular MP4, and renaming .m4v to .mp4 usually just works. The extension is mostly a convention signaling "this was made by Apple" or "iTunes Store provenance".
