The .doc file dates all the way back to Microsoft Word 1.0 for MS-DOS (October 1983). For the first decade it was a simple binary stream; then Word 97 (1997) switched to the OLE Compound File format — essentially a miniature filesystem inside a single file. That structure allowed Word to embed other Office objects (Excel sheets, charts, drawings) as first-class citizens while staying backward-compatible.
For 24 years .doc ruled the office. Its grip loosened in 2007 when Word adopted DOCX as default, and today .doc is mostly a legacy fallback — you still encounter it in government archives, law firms with old systems, and emails from the user who installed Office 2003 and never updated. The format remains supported in every Word version for compatibility, but Microsoft stopped improving it in 2007.
