DPI metadata from the ICNS is used to compute the image's on-page size in the PDF. A 300-DPI photo of a 4x6 receipt lands as a 4x6-inch image on the page; override with "fit to page" in Advanced if you want it to fill the page instead.

- Set page size (A4 / Letter / custom) in Advanced to match the destination printer's default, avoiding margin surprises.

Yes. The free tier accepts files up to 25 MB without registration, email capture or watermarks; paid plans go up to 2 GB. Paid plans raise the size cap, enable batch conversions and provide a REST API for automation, but nothing on the free tier is quality-limited — the output is exactly the same as on any paid plan.
